new patients

• Contact your physician or medical provider to let them know you would like to be referred
• Have your physician send a referral letter, or use our referral form, and medical records to either our fax at 307-670-8250 or mail to our Gillette office address
• Send us an email to office@awyomingpainclinic.com to request a new patient packet and referral form for your provider
• After we receive your referral, we will review your records and call you with an appointment
• To get a head start on your care, you may use the Portal on this website
• OR print out the new patient packet, complete and mail, email or fax back to our office

Our office is based out of Gillette however, we do a satellite clinic in Casper Wyoming.  Please make sure your sending any correspondence to the Gillette Clinic address as we are only in Casper on limited clinic days.
